Secure the Enrollment Route Form and Application Form from the Admission Unit or Print our Downloadable Forms.
Verification
Submit PSA Birth Certificate, SHS Report Card, and Good Moral for checking.
Clearance
Complete Medical assessment and Student Affairs guidance clearance.
Academic Advising
Proceed to your College for assessment and system enrollment.
Final Submission
Stamp Registration forms at the Cashier and submit to the Registrar.
